Liability Release Form

I, the undersigned, owner or authorized agent for ________________(pet’s name)do hereby release Anderson’s Pet Express and it’s employees or agents of any and allresponsibility, financial or otherwise as well as damage or loss of property.

Anderson's Pet Express cannot assume any responsibility for injury, property damage or loss of life due to outside distractions and/or unleashed animals. I assume full financial responsibility for any and all such injury, damage or loss that may occur.

I agree to pay any and all charges incurred with connection to a claim/lawsuit including legal and/or court costs.

I hereby certify that I have read and fully understand this release.
